Surgery：Dog Spay

今天手術是母狗結紮。
這禮拜我輪到的是觀察員，齁齁，
不過要負責記筆記，
要把手術步驟過程寫下來。
（內有血腥畫面，慎入！）

 

Priscilla，打完BAA（IM）的樣子，好像快哭了
聽說她照X光的時候很不乖，
我同學的手指還被她的牽繩纏住刮傷，
很深很長的一條血痕。
所以就沒有照了（因為沒禁食也不能打麻醉）
不過血液尿液檢查都正常，
有寄生蟲，Coccidia，在吃驅蟲藥。

 

 

打完Ket-val（IV）
導管已經放好了，後來發現點滴沒在跑，
檢查才知道原來導管被折到，
結果老師很神奇地把它弄直又放回去哩。

 

 

可愛的腳腳
牠長得很討喜，四隻腳上都有白毛，
有好笑的鬍鬚，鼻子還有一半是白白的。

附上我做的筆記： 

March 7 Wednesday    Priscilla (canine spay)

 

2:05 – BAA given

2:25 – catheter in

2:30 – ket-val given

2:32 – endotracheal tube in, O2 2 L

2:36 -   O2 turn down to 1 L

2:45 – isoflurane turn down to 2.5%

3:20 – move to surgery room

3:25 – final preparation

3:25 – four ground drapes on

3:27 – towel clamps on the ground drapes

3:29 – lab drape on the incision site

 

3:30 – use #20 scalpel blade to cut along linea alba 

              through skin

          - clean blood with gauze square

          - use Metzenbaum scissors and rat tooth forcep  to make the incision bigger

            and deeper (cut through tissue)

          - clean blood with gauze square

          - use spay hook to find the uterus (fingers in to feel the intestine)

    - found ovarian ligament, break it by fingers

    - one Carmalt forcep clamp on the cranial end of uterus horn

    - 2-0 vicryl and gold handle needle driver, suture the cranial end of uterus horn

    - one curved Carmalt forcep clamp on top of the first one

    - use #20 scalpel blade to cut between the two Carmalt forceps

    - find the other uterus and repeat the procedure

    - use Metzenbaum scissors cut on uterus to make it to be a big Y shape open

    - pull out the uterus horn joint

    - put one straight Kelly forcep on the base of the joint

    - 2-0 vicryl suture on the base – as tight as possible

    - one straight Calmalt forcep on Kelly

    - use #20 scalpel blade to cut between the two forceps – both uterus and ovary out

    - Allis forcep clamp on the base of the joint

      - check bleeding

      - take Allis off

      - clean the blood

    - use rat tooth forcep and 2-0 vicryl start to close the incision (the inside layer)

4:00 – turn isoflurane down to 1%

    - clean blood

    - one curved mosquito forcep clamp on the caudal end of the inner layer

     - check bleeding

    - use 2-0 supramid suture on outer layer (skin)

4:17 – isoflurane turn off    

    - surgery finished
